Bipolar disorder (BD) is a severe psychiatric disease that contributes to a significant worldwide burden of disease. Its etiology unknown and clinical diagnosis and treatment are difficult. This special topic brings together reviews and insights on the treatment, management, and mechanisms of BD. Several original studies report neuroimaging and genetic changes in BD patients. In this cover image, the circle is made up of a brain half and a vascular half. In the middle of the circle, the two tissues are connected by astrocytes to form the blood-brain barrier, representing a possible neuroimmunological mechanism of the etiology of BD. The division of the circle adopts a structure that similar to tai-chi to represent the completely opposite clinical symptoms in BD patients. (Cover image provided by Zhiang Niu and Lvchun Cui)
