Deep brain stimulation (DBS) is emerging as a potential strategy for restoring arousal and functional communication in carefully selected patients with disorders of consciousness. Zang et al. integrate mesocircuit mechanisms, connectome-guided targeting, cognitive–motor dissociation, and adaptive neuromodulation to chart a path from experimental stimulation toward circuit-guided precision therapy. On the cover, a mist-shrouded pine forest forms the folds of the cerebral cortex, evoking a brain whose pathways of communication have become obscured after severe injury. At its center, paired luminous roots symbolize the central thalamus, while a warm beam of DBS light represents re-engagement of the anterior forebrain mesocircuit. Firefly-like signals trace a narrow route through the forest, representing renewed cortico-striato-thalamo-cortical communication. The solitary figure following this path embodies covert consciousness and the cautious possibility of re-emerging from silence into meaningful interaction. See pages 2057–2075.
